I understand that I am required to present a 15 minute tape of my work with a couple at one of the modules in order for this training to count towards my EFT certification. *

I understand if i don't present a case, I will have to make it up in a supervision group with a trainer or supervisor at a later date, for the cost of the supervisior group. *

I am sending a non-refundable deposit of $200 (etransfer is fine) with this registration form to hold my spot. *

I understand that I am making a financial commitment to all four modules. A total cost of $1600 plus HST for a total of $1808.00. *

I understand that I need to send four posted cheques made out to R and R Associates and mailed to 28 Regal Road, Toronto, ON M6H 2J7 to be cashed one month before each module (Sept. 4/$452, Nov/$452,Dec.31/$452 and Feb.27/$252. *
